 Venous Air Embolism
During spontaneous respiration, slow entrainment of air that causes obstruction of 10% of the pulmonary circulation causes a "gasp" reflex that results in chest pain and tachypnea.
If a large volume of air has been entrained, and surgical conditions permit (i.e., the head is not in pins), positioning the patient in the left lateral decubitus position will help to keep air in the right atrium from entering the ventricle.
Placement of the transducer may be difficult on some patients, especially those with a chest wall deformity or who are obese.

Embolism - Wikipedia, the free encyclopedia
In medicine, an embolism occurs when an object (the embolus, plural emboli) migrates from one part of the body (through circulation) and cause(s) a blockage (occlusion) of a blood vessel in another part of the body.
Emboli often have more serious consequences when they occur in the so-called "end-circulation": areas of the body that have no redundant blood supply, such as the brain, heart, and lungs.
This can be contrasted with a "thrombus" which is the formation of a clot within a blood vessel, rather than being carried from elsewhere.

MASSIVE SYSTEMIC AIR EMBOLISM FOLLOWING A BLUNT CHEST INJURY
Needle venting of air from the heart by the surgeon, manual cardiac massage, and pressor therapy to increase coronary perfusion pressure beside right decubitus position are standard cardiopulmonary resuscitative measures for the victims of left heart air embolism.
The initiation of systemic air embolism in a patient with chest trauma after mechanical ventilation is attributed to bronchopulmonary pressure gradient that favors transmission of air through a traumatic bronchial to pulmonary venous communication, and this has been proved by experimental study on animals.
Systemic air embolism is often noticed after the initiation of mechanical ventilation, so anesthesiologists must be aware that the patient with chest trauma is at risk of systemic air embolism during mechanical ventilation, especially with high-peak airway pressure.

Air Embolism during Calibration of Invasive Blood Pressure Monitoring Systems
There are methods of applying reference pressures for those special circumstances that minimize the risk of air embolism.
However, this calibration pressure may not be adequate for confirmation of high systolic pressures, and the technique may be awkward in some settings.
(For example, a small amount of air can be introduced into the system, even with the patient stopcock closed during calibration, if the calibration pressure exceeds that of the pressure infuser flushing system.

Nursing: Managing a venous air embolism
A patient with a venous air embolism will exhibit respiratory distress, increased heart rate, cyanosis, a drop in BP, and a sudden change in level of consciousness and may complain of back or shoulder pain.
The physician may aspirate the air from the right atrium using an intracardiac needle.
If the patient coughs, sneezes, cries, laughs, or takes a deep breath, the change in intrathoracic pressure can draw 10 to IS cc of air into the venous system as a bolus.
